Media Checkout Information & Reservation Form

Reservation Process

  • Access the ITS online Media Catalog to obtain program information, catalog number, program length, format, and availability.
  • Use the form below to make a reservation.
  • Pick up the materials at the Checkout Counter (AH-1129). A current faculty or staff identification card is required.
  • All materials are to be used for instructional purposes only and are not available for personal use.
  • Instruction is provided on the use of materials if necessary.
  • Remote controls are available for most TV sets and may be obtained from your department office or the Checkout Counter (AH-1129).
  • To learn more about Media Reservations, please contact the Checkout Counter at x45691 or checkout@mail.sdsu.edu.
